Candidate Review — the Human Gate. AI extracts the candidate. The reviewer sees the full title in serif, the source meeting, and linked evidence context. Three choices: Reject, Request more evidence, or Approve & commit to durable memory. If a prior decision is affected, a re-decision warning appears before approving.

Conflicts & Re-decisions. When a new candidate touches or reopens a prior approved decision, DM surfaces it automatically — side-by-side with the superseded record. Re-decisions, conflict signals, and resolved supersessions are tracked in lineage so the full history is always queryable.
